Purpose of the guideline: | Due to the particularity of anatomy, tumors occurring in the pelvis and sacrum are often deep, and the early symptoms of patients are not obvious, and most of them are detected when the volume is larger. The blood supply of tumors is usually rich, and the tumors are close to key vessels, nerves and internal organs and other structures, so the surgical resection and reconstruction are very difficult, and the huge surgical risk makes many orthopedic surgeons shy away. Among them, massive bleeding is the main problem in the operation. In order to solve the problem of massive bleeding, temporary balloon blocking technique of abdominal aorta has been tried in recent years for hemostasis in sacral and pelvic tumor resection. Due to the large population base in China and the large number of patients with sacral and pelvic tumors, many institutions have gradually applied abdominal aortic balloon surgery, and have rich experience in use. Despite this status, there is no broad consensus on the specific techniques of balloon occlusion of the abdominal aorta in pelvic and sacral tumor resection, including balloon insertion, duration and frequency of aortic occlusion, and management of balloon-related complications of the abdominal aorta. To this end, the Professional Committee of Bone Tumors and bone metastases of the Chinese Anti-Cancer Association organized relevant domestic experts to select the clinical issues that bone oncologists are most concerned about, and finally formed recommendations. This expert consensus is based on the joint discussion of domestic experts in bone tumor, interventional radiology, vascular surgery and other fields. Its purpose is to promote the application of temporary abdominal aortic balloon blockade, improve the safety level of this technique, and reduce the probability of complications caused by it. |
