指南制订的目的: 百日咳是由百日咳鲍徳特菌感染引起的、严重威胁儿童健康的急性呼吸道传染病。本病曾因疫苗的普及全球发病率降至很低水平。但近年来,世界各地又出现“百日咳再现”问题,中国近2年的发病率出现大幅度上升,达到近30年的最高水平,重症率和死亡率也明显增高,严重威胁儿童健康。中国的百日咳菌株对历史沿袭首选药物大环内酯类耐药率高,耐药常导致治疗失败。另一方面,疫苗时代,症状不典型的百日咳患者也明显增多。为帮助广大儿科医师、全科医师更好认识新形势下儿童百日咳的疾病特点,规范疾病诊断流程,推荐抗生素选择策略以及重者患者的救治措施,特制定本专家共识。
Purpose of the guideline: Pertussis, also known as whooping cough, is a highly transmissible respiratory disease that is caused by the bacterium Bordetella pertussis (B. pertussis). Due to immunity attenuation and B. pertussis mutations after vaccination, pertussis resurgence has been reported in many developed countries with high vaccination rates. In the vaccine era, the burden of pertussis has shifted from infants to adolescents and adults in many developed countries, and they became the main source of infection for infants. According to the law of the People's Republic of China on the Prevention and Control of Infectious Diseases, pertussis is classified as a Class B infectious disease. Over the past decade, pertussis resurgence in China has attracted attention. Pertussis cases reported in China are still dominated by infant cases, and the bacterial isolates are generally resistant to macrolide antibiotics, which are different epidemiology from other countries and make some troubles for clinical management. In order to further standardize the clinical diagnosis and treatment of pertussis, this diagnosis and treatment plan was formulated based on the research progress and clinical experience at home and abroad.
